In an interview to be found here , Charles Sappington said:
Q: Did you create the image for the album cover? Or just the design?
A: I created the package. However, the deal was at the time … I promised them I wouldn’t talk about it. There was a reason, and it was legal. They had some legal problems with that cover. I suspect enough time has passed, but I have to stick to my word unless I get approval from the Dylan camp. (Note: Dylan’s camp had no comment.) What I can say is that Bob Dylan supplied the original image and then we distorted it from there.
Q: Did you have much interaction with Dylan on the cover?
A: They originally had a photographer shoot some photos of Dylan and Tom Petty. I heard Dylan took a look and threw them all in the trash. The only thing he liked from the shoot was a Polaroid test shot, which is the first thing they gave me. I fiddled with that, but they didn’t care for it, and we went in a different direction. That’s the part I can’t talk about. But on the inside there were the thank-yous …
Q: There were a ton of thank-yous …
A: Yes, but it didn’t start out that long. It was about 20 names. Bob told me he didn’t want it in this typeset. He wanted it hand-lettered but not calligraphy. I knew exactly what to do. I turned it over to my now ex-wife. She was at UCLA in a landscape architecture program. She was into lettering on drawings, which I thought would be perfect. I sent Bob a sample, and he liked it. But every morning he called with new names to add to the list. The morning it had to go to press he called with five more names. My wife was headed out the door to go to her job, and I said, “Wait, you have to do this before you leave.” She sat down and did it.
The Knocked Out Loaded version of Brownsville Girl appears on 1994's Greatest Hits Vol. III, but now been superseded by an alternate mix that first appeared on the 2007 Columbia/Legacy compilation Dylan and again on the US Columbia/Legacy compilations Playlist: The Very Best Of Bob Dylan '70s (2010) and The Real... Bob Dylan: The Ultimate Bob Dylan Collection (2012), for both see International Album Releases (Dylan-Only Compilations) 2010s.

The commercial cassette release has a folded insert with the tracklist and musician credits on the reverse. The cassette is light grey with black text, there are no paper labels. Got My Mind Made Up is again wrongly titled "I've Got My Mind Made Up" on both sides of the cassette insert and Side 2 of the cassette.

This CBS release was made in England. It has a three-fold front insert and a black cassette with red CBS paper labels with black text.
Columbia 467040 4 (UK), 1990s (two releases):
These Columbia releases were again made in England. They have the same three-fold front insert as the 1980s cassette release, but the cassettes have the Columbia logo. This time the cassette is grey with black text printed directly onto it, there are no paper labels. The second release has exactly the same insert but the cassette is smoked transparent grey with white text.

This album was made in Israel. The front sleeve has the catalogue number at top left and white Hebrew text below "BOB DYLAN" meaning "New Album!". The rear sleeve has a yellow panel with text in Hebrew and English. The record has red CBS labels. There is an insert with text in Hebrew. Thanks to Larry Yudelson for the information that the insert is not Bob-related, but is instead promoting Tel Aviv-based Israeli singer Tammy Leviav, photo above. The copy shown has matrix numbers: Side 1 - PAL-40439-1B (handwritten) 86326-AQ (stamped), Side 2 - PBL-40439-1A (handwritten) 86326-BQ (stamped).

The promo copy shown does not have an obi, as is normal for Japanese promo releases. The rear sleeve has a "Sample" sticker, a barcode and Japanese copyright text. The rear sleeve of both promo and commercial copies is dated "H-9-5" (5 Sep 1986). The promo record labels have the usual Japanese characters indicating a promo to the right of the centre hole above the catalogue number. The record of Manuel García Jara's promo copy has stamped matrix numbers: Side 1 - "28AP-3224A1 1A1 @CS", Side 2: "28AP-3224B1". Hans Seegers has a promo copy with a sample sticker on the rear sleeve as illustrated. The record has red labels with promo stamps, and stamped matrix numbers: Side 1 - "28AP-3224A2 1A1 @ CS", Side 2 - "28AP-3224B1 1A4" (different from those of Manuel's promo copy).
The commercial release of this album has a yellow top obi with black test and price ¥2,800 (the "28" in the catalogue number indicates the price). Both the promo and the commercial copies have a double sided insert with Japanese text on one side and Dylan discography on the other and a second folded four page insert in English and Japanese, the front and rear of which are the inner sleeve designs used elsewhere.
This appears to have been the last Bob Dylan album released on vinyl in Japan. Even today with the resurgence of vinyl, LP releases sold in Japan are imported from the USA or Europe.

This album was made in the Philippines by OctoArts International Inc. for CBS Records. As usual for the Philippines, the titles of four tracks have been added to the front sleeve, this time in white below "BOB DYLAN". The front sleeve also has the catalogue number in very small text at bottom left. The rear sleeve has credits printed on the left edge. This album can be seen to have been a sample because it has a punched hole at top right of the front sleeve and a promo sticker that covered the barcode has been partly removed on the rear sleeve. The record has orange/yellow CBS labels and the copy shown has matrix numbers: Side 1 - P (P) PAL-40439-1B QCL-22251-A, Side 2 - P (P) PBL-40439-1B QCL-22251-B.

The record has orange/yellow CBS labels, surprisingly still with the name of The Gramophone Company, the previous manufacturers for CBS in South Africa and Zimbabwe. The record of this copy has handwritten matrix numbers: Side 1 - ASF 3114 A, Side 2 - ASF 3114 B.
Lars M. Banke has a copy of this record in a ZMC (Zimbabwe Music Corporation) sleeve. The ZMC sleeve is the same on both sides. ZMC made the subsequent Zimbabwe releases up to Under The Red Sky in 1990. There are no promo stamps on the record labels and they are exactly as shown for the commercial release. The record of this copy has the same handwritten matrix numbers as the commercial copy.

The picture from www.eil.com also has a purple/white sticker on the album sleeve which looks like a record shop label and isn't present on my copy. The record comes in the US inner sleeve with differences as shown. has red/yellow labels with written matrix numbers: Side 1 - 0211-17009-1A, Side 2 -0211-17008-1B.
The song titles are translated into Spanish on the cover and on the labels. You Wanna Ramble and Under Your Spell are translated here as "Quieres Pasear" and "Hechizado" while in Costa Rica they were translated as "Quieres Aventurar" and "Bajo Tu Hechizo" (see US & International 7" & 12" Stereo Singles & EPs 1984-86). Others are "Ellos Lo Mataron" [They Killed Him]; "A La Deriva Lejos De la Playa" [Driftin' Too Far from Shore]; Preciosos Recuerdos [Precious Memories]; "Quiza Agun Dia" [Maybe Someday]; "La Chica De Brownsville" [Brownsville Girl]; "Estoy Recuperado" [Got My Mind Made Up]. Lars M. Banke has noticed that on the rear sleeve and on the Side 1 label They Killed Him is incorrectly listed as “They Kill Him”.

The promo copy of this album has a prominent slanted red stamp on the front sleeve with the words "Disco Para Demostracion - Venda Prohibita" [Demonstration Disc - Not For Sale". These words are also printed on the record labels on either side of the CBS logo above the "Discos CBS" text. The song titles are translated into Spanish on the rear sleeve and record labels. All are good translations (says Sergio Magnacca) except "Got My Mind Made Up" which is clumsily translated as "Cambie Mi Forma De Pensar" which means "Changed My Way Of Thinking" (surprisingly similar to the title from Slow Train Coming). You Wanna Ramble is also translated here as "Quieres Pasear" (see the entry for Mexico above). The record has orange/yellow Discos CBS labels and this copy has matrix numbers: Side 1 - AL 40.439, Side 2 -BL 40.439. Surprisingly, the front sleeve has the Columbia logo at bottom right.
The commercial version has the same front and rear sleeve without the red promo stamp. The orange/yellow Discos CBS labels are the same, again without the promo text, and the record of this copy has the same stamped matrix numbers as the promo copy, but also has “© 6” stamped on both sides.

These extremely rare stereo cassette releases were made in Chile by CBS Records Chile Ltda. The inserts of both releases shown look identical apart from the second release insert appears to have been printed in stronger colours. The song titles on the cassette insert and labels are in English only. The first release cassette is light grey with white CBS labels and orange text. The second release cassette is again light grey with black text printed directly on to it, there are no paper labels.

This very rare release also has the song titles (in English) on the rear sleeve. The sleeve and record were made in Venezuela. The record has red CBS labels with handwritten matrix numbers: Side 1 - CS 10547-A, Side 2 - CS 10547-B. The labels have track titles in English and Spanish (again with varying translations) and the text "CBS/USA" to the left of the centre holes.

This very rare release has the same catalogue number as the US release and a standard US Columbia sleeve, including the bar code. The record has cream CBS labels with blue and black text and was manufactured and distributed by Mu&icor C.X.A.

CD Releases
"Knocked Out Loaded" - CD releases, CBS/Sony 32DP 499 (Japan), 21 Sep 1986, re-released 1989, Sony Records SRCS 6341 (Japan), 22 Sep 1993:
First release: CBS/Sony 32DP 499, 21 Sep 1986: This release has a orange and white obi with black and red panels with black, white and dark red text and price ¥3,200 (this was before sale tax was introduced in Japan- the "32" in the catalogue number indicates the price). The front insert is a 24-page stapled booklet.
Second release: CBS/Sony 32DP 499, 1989: This release is the same as the first release with the orange and white obi, this time with price ¥3,008 (¥2,920 plus sales tax).
Third release (mid-price): Sony Records SRCS 6341, 22 Sep 1993: This release has a black, red, white, blue and yellow obi with price ¥1,800 (¥1,748 plus sale tax). The release date is "93-9-22" (23 Sep 1993) and the withdrawal date is "95-9-21" (21 Sep 1995). The original Japanese release date is shown as "84-8-24" (24 Aug 1984). The copy shown is a promo, with a numbered promo sticker on the rear insert over the barcode, and Japanese characters indicating a sample in the clear centre of the CD.

Gerd Rundel's later CBS release came in a red CD tray, again with an eight-page booklet as the front insert. The rear insert has "CBS" at top right and different copyright text at the bottom. The CD was now made in Austria with red panels. It has an unusual CBS catalogue number "467040 2" to the right of the centre hole, the expected "CDCBS" prefix is missing.

This massive set is the latest in a series of "Complete Columbia Album Collections" featuring artists such as Miles Davis, Charlie Mingus, Duke Ellington, Dave Brubeck and even Blue Oyster Cult. The CD version of the set includes 41 Dylan albums in card sleeves, 14 of which have been remastered for this project, plus a 2CD set of previously released non-album tracks entitled Side Tracks. The albums exclude all compilations and The Bootleg Series sets, but do include the deleted 1973 Dylan album, also see 1973, and the extra 2CD set Side Tracks includes one side of the 1971 single George Jackson (Acoustic version), R-0085, see 1971. The set is therefore listed in 2013. The set also includes a hard cover book with extensive new liner notes and previously unseen photos.
All the CDs come in card sleeves. The European sleeves are made of thicker card than the US sleeves, but they are otherwise identical, so are not shown. The catalogue number on the sleeve is "88691924312JK28" plus "DISC 32". This album has been remastered, so is the tenth Dylan album remastered for this project. Thanks to Georg Thoma for reminding me that the version of Brownsville Girl on the 2007 3CD compilation Dylan is a different mix from that of the original 1986 album and that mix appears again on the 2010 US Columbia/Legacy compilation Playlist: The Very Best Of Bob Dylan '70s, see International Album Releases (Dylan-Only Compilations) 2010s. This CD has a remastered version of the original 1986 mix. The CDs are different in detail, the US Knocked Out Loaded CD has catalogue number "88691924312D32", whereas the European CD has catalogue number "88691924312-32". The European CD was made in the EU (probably Austria). The US and European copyright details are printed at the bottom of the Bob Dylan CDs, and round the circumference for The Freewheelin' Bob Dylan onwards, so I haven't shown them for the later CDs.
